SMART: Sensory Motor Arousal Regulation Treatment Manual

Second Edition, with a new chapter on "Caregiver Involvement in Treatment", published July, 2014.

A Manual for Therapists Working With Children and Adolescents: A "Bottom Up" Approach to Treatment of Complex Trauma

by Elizabeth Warner, Psy.D., Alexandra Cook, Ph.D., Anne Westcott, LICSW, Jane Koomar, Ph.D., OTR/L, FAOTA

The SMART manual presents a new model of psychotherapy for traumatized children and adolescents designed to be useful for mental health clinicians in clinics, residential treatment sites, and schools. The model draws on knowledge from sensory integration occupational therapy, body-oriented therapies, especially Sensorimotor Psychotherapy, and the collective wisdom of the authors, participating clinicians, and the families who joined us in its development.

SMART is designed to treat the problems of regulation common to these children and adolescents through an active therapy in a room which allows for movement and play. Photographs in the manual give a visual image of how the equipment in the room may be used, and a framework for understanding is provided. The tools of regulation and clinical skills that support the treatment model are spelled out, as well as the model of regulation and trauma processing that occurs naturally and leads to healing for child and family.

In the Second Edition, a new chapter, entitled "Caregiver Involvement in Treatment" provides guidance for therapists on the considerations for engaging caaregivers in a SMART therapy, including caregiver factors and skills, the range of options for caregiver participation, and roles that a therapist may play with the caregiver over time. Finally, the creation of playful "rhythms of engagement" through use of the SMART tools of regulation is described.
